Responsibilities Evaluate patients within twenty-four (24) hours or one business day of physician referral. If unable to complete evaluation within 24 hours, document in the medical record the cause for delay. Develop effective treatment plans and obtain approval for services from the referring physician. Treat patients according to the physician treatment plan. Assist the nursing department with training of Restorative Aides and supervise Physical Therapy Assistants in direct patient care and patient‑related activities, following the state practice act. Communicate with the supervisor and health team members regarding patient progress, problems and plans. Participate in Patient Care Conferences, Utilization Review meetings and Rehabilitation Conferences as needed. Participate in the in‑services training program for other staff in the facility. Record evaluations, daily treatment notes, weekly progress notes and recertification documentation per PT Board State Practice Act and governmental and third‑party payer requirements. Record treatment changes per policy and procedures. Instruct patients’ families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services. Participate in discharge planning. Recommend and facilitate ordering of necessary durable medical equipment to facilitate patient independence in mobility (e.g., braces, walkers, canes, elevated toilet seats, grab bars). Report any problems with department equipment so it is maintained in good working order. Maintain positive, effective, and appropriate relationships with patients/residents, families, staff and professional colleagues. Supervisory Requirements Supervises PT assistants, aides and students. Qualifications Bachelor’s degree in Physical Therapy; preference for graduates of a master’s or doctorate program in Physical Therapy. Proficient in computer skills such as inputting data into Casamba and Point Click Care. Ability to read technical procedures and policy and procedure manuals. Ability to effectively present information and respond to questions from managers, coworkers and families. Mathematical skills: apply concepts such as fractions, percentages, ratios and proportions to practical situations. Reasoning ability: solve practical problems and interpret a variety of written, oral, diagrammatic or schedule instructions. Certificates, Licenses, and Registrations Licensed as a Physical Therapist in the state. Must meet continuing education requirements per state practices. Maintain a license in good standing at all times with the state board. Physical Demands Essential functions require the following physical abilities: standing and/or walking very frequently; sitting occasionally; reaching with hands and arms frequently; pushing or pulling very frequently; talking or hearing very frequently; tasting or smelling very frequently; exerting in excess of 100 pounds of force occasionally or 50 pounds of force frequently, or 20 pounds of force constantly to move objects; climbing, balancing, stooping, kneeling, crouching or crawling occasionally; visual acuity for close, distance, color, peripheral and depth perception, with the ability to adjust focus.
